Aerial Workshop

I, (participant) recognise that the use of the Aerial Acrobatic equipment can be dangerous and acknowledge the risks involved and that the organisers and Trinity Stage School and all companies involved have taken all reasonable and practical steps to minimise the risks.

I will wear appropriate clothing and use any protective equipment provided or advised.

I will not undertake any manoeuvres on the Aerial equipment that are beyond my level of skill or are a danger to others.

I declare I am in good health and not suffering from any medical condition or disability which is likely to adversely affect my normal control or which might make it unsafe for me to participate.

I confirm that I will be responsible for the payment of any damage caused as a consequence of my dangerous manoeuvres, loss of control, or negligence.

I fully indemnify the organisers and Trinity Stage School and all companies involved with the Trinity Stage School feature against all and any loss or claims howsoever arising as a consequence of my participation on the Trinity Stage School Aerial equipment.

I fully indemnify the organisers and Trinity Stage School and all companies involved with  Trinity Stage School against claims of injury to myself or any third party howsoever arising as a consequence of my participation on the Aerial acrobatic equipment .

This agreement is covered by English Law. In the event that any part of this agreement is for any reason unenforceable the remained shall remain in full force and effect.

*I have read, understood, and agree with the Code of Conduct as displayed.

*For persons under 18 years: I agree, and being the lawful parent or guardian of the ticket purchased for and having read and understood the terms of registration hereby agree the by way of approval and undertake acceptance of the above on behalf of the participant.
